POSITION TYPE                           INDEFINITE TERM, FULL-TIME, PART-TIME (10 – Hour Shifts)(Days, Evenings, Occasional Overnights)

 Position Summary:

The Mobile Assist and Connect Team Member (MAC247) / Case Coordinator will engage and connect with Indigenous individuals, specifically women who require supports, and provide referrals as needed. Safety concerns around intimate partner violence, exploitation and overall safety will be a priority. In addition, the goal of the Mobile Assist and Connect Team Member (MAC247) / Case Coordinator is to help individuals access social services and other community resources; essentially a one-stop information resource for available programming.

The DCSP is committed to working with the Indigenous-led organizations integral to ensuring that outreach staff continues to reflect the community they serve. Hiring and training policies explicitly privilege the role of Indigenous lenses, knowledge keeping, and identities to facilitate authentic, healing relationships with the community. The Mobile Assist and Connect Team Member (MAC247) / Case Coordinator will provide high level street presence to support existing operations. This will allow for “point of contact” interactions and engagement with the community. They will assess, triage and provide continued and consistent support with social service navigation and case management. Cultural, spiritual and holistic supports will be prioritized.

As part of the existing DCSP organizational framework, the Mobile Assist and Connect Team Member (MAC247) / Case Coordinator will have additional supports from DCSP MAC 24/7 (Mobile Assist and Connect), CONNECT, COAR (Community Outreach, Advocacy and Resource) and CAN (Coordinated Assistance Network) teams.

Key Responsibilities:

  • Identify needs of individuals who are vulnerable or at risk of crisis that may require immediate emergency support and/or referral to Mobile Assist Connect Team (MAC247) or Community Outreach Advocacy Resource (COAR) based on the circumstances.
  • Support participants by providing short-term supports and connecting them to social services agencies.
  • Assist with the development of individualized service plans in collaboration with participants.
  • Act as an advocate for the current needs of participants.
  • Liaise to build positive working relationships with social service agencies in the community.
  • Develop and maintain professional and supportive relationships with participants through a trauma informed lens.
  • Proactively identify barriers that restrict participants.
  • Compile and maintain records and statistical data in a timely manner, for the purpose of producing reports using an internal database and in turn, external partners as required.

Qualifications:

  • Related or prior work, lived and/or volunteer experience
  • Minimum one year working experience with vulnerable sector
  • Knowledge and understanding about trauma informed approach, motivational interviewing, and strengths-based approach
  • Certification in First Aid, MHFA, ASIST, NVCI is an asset
  • Experience working on case management database platform is an asset
  • Proficiency in Microsoft Office Suite
  • Experience in Non-profit sector is an asset
  • Physical ability to be on foot and perform the duties of the position (which may include standing or walking for extended periods of time).
  • Class 5 driver's license preferred.
  • This role is subject to a Criminal Record Check, including Vulnerable Sector Search, and an Adult and Child Abuse Registry Check, to be produced prior to commencing employment
