POSITION TYPE  FULL-TIME

Position Summary:

The Service Facilitator has operational responsibility for the Community Stabilization Service (CSS) and plays a critical role in ensuring that the CSS succeeds as a short-term, interdisciplinary consultative service to people with intellectual disabilities whose living arrangements are at risk of breakdown due to complex challenging behaviours or other reasons.

Using a strengths-based, person- and family-centered approach, the Service Facilitator promotes a positive environment for the individual and works collaboratively to support formal and informal networks, and enhance knowledge, competencies and skills. The Service Facilitator takes an active role in promoting services and developing creative strategies to work with individuals, families, external service providers and systems, as required.

The Service Facilitator is responsible for scheduling and chairing interdisciplinary meetings, placing referrals for additional clinical services as required, and otherwise ensuring that services are delivered in a timely and time limited fashion. The Service Facilitator is responsible for coordinating education and training delivered by CSS team members, and works collaboratively with Community Support Workers to achieve positive outcomes that contribute to the person’s dignity, quality of life and self-determination.

Qualifications:

  • Bachelor of Social Work degree, a Bachelor’s Degree in a human services field or an acceptable combination of education and work experience.
  • A minimum of 4 years’ work experience in the area of developmental disabilities is preferred and is an asset.
  • A minimum of 2 years work experience providing services to people with disabilities in a community setting is strongly preferred.
  • Demonstrated knowledge and experience using a person-centered model of service provision.
  • Knowledge of the history of services to people with disabilities, current trends, philosophies and practices concerning their care, education and lifestyles.
  • Knowledge of relevant legislation including the Vulnerable Persons Act, Accessibility for Manitobans Act, and the Personal Health Information Act.
  • Non-violent crisis intervention (NVCI) training is an asset.
  • Knowledge of psychosocial and environmental factors involved in chronic disability is an asset.
  • Working knowledge of service systems and ability to creatively access or develop informal and formal supports or resources.
  • Ability to communicate openly and appropriately using positive interpersonal communication skills.
  • Ability to engage in a strategic and diplomatic manner with complex support networks.
  • Ability to organize, chair and conduct meetings.
  • Ability to understand, support and practice interdisciplinary teamwork.
  • Able to effectively prioritize, structure and manage responsibilities to achieve targeted outcomes.
  • Positive, energetic, self-directed and client-focused.
  • Possess excellent written and verbal communication skills.
  • Must possess a valid Manitoba driver’s license and have access to a vehicle.
