JOB SUMMARY
In the Transition Services team we are guided by values that centre on providing person-centered, strengths-based, trauma-informed care rooted in harm reduction and the Housing First approach.
Guided by Siloam Mission’s Strategic Plan, the Transition Services Case Worker is responsible for providing case management and housing location services to persons experiencing chronic or episodic homelessness, as well as those requiring housing loss prevention. The Transition Services Case Worker will partner with various programs within Siloam Mission and the community to streamline and administer individualized services. In addition, they will also coordinate and participate in assessments and crisis intervention, and provide access to social services, supportive counselling and referrals.
KEY DUTIES & RESPONSIBILITIES
Housing Support
- Prioritize locating, securing, and maintaining permanent housing with participants, in alignment with Housing First fidelity standards;
- Provide strength-based case management and service coordination to support long-term housing stability with eviction prevention;
- Educate participants on their tenant rights and responsibilities;
- Support participants with unit maintenance and cleanliness, including preparing for pest treatment, assisting with deep cleaning, and addressing hoarding and collecting behaviors, etc.;
- When necessary, arrange and assist participants with housing-related moves;
- Establish and maintain partnerships with landlords, and advocate for participants as needed;
Case Management & Service Coordination
- Interact with participants using a trauma-informed approach to obtain history and assess relevant skills and strengths;
- Collaborate with participants to develop their individualized service plans addressing short-term and long-term goals;
- Support participants in connecting with and accessing services and supports in areas such as legal, medical and financial assistance, housing, employment, transportation, family services, etc.;
- In collaboration with participants, team members, supervisors and external stakeholders, provide ongoing case management to assess, monitor, and communicate participant progress, ensuring service plans are met or adapted as needed;
- Proactively engage with participants in the community and their homes—without coercion—to support their transition into housing, strengthen support networks, locating individuals as needed (e.g., shelters or frequented areas)
- Act as an advocate for the needs of the participants of Siloam Mission as required;
- Offer connections to culturally appropriate services or supports, both internally and externally;
- Participate in field trips with program participants & graduates, including at least 2 overnight camping trips per year;
Administrative Duties
- Prepare and complete intake documentation in accordance with program standards;
- Maintain electronic and paper participant records in accordance with department procedures;
- Timely contribution to outcomes reporting for funding agreements and operational directives, as required;
- Attend and participate in Transition Services and Siloam Mission meetings and events as required;
- Participate in professional development opportunities;
- Support the learning and integration of student interns through guidance and mentorship, as required;
- Other duties assigned as relevant to the position;
EDUCATION & EXPERIENCE
- Completion of a diploma/degree in a health or human service such as social work, psychology. Combination of education and experience may be considered;
- Certification in or demonstrated knowledge of NVCI (Nonviolent Crisis Intervention), ASIST (Applied Suicide Intervention Skills Training), CPR/First Aid, Mental Health First Aid (MHFA), Motivational Interviewing is an asset;
- Experience and demonstrated knowledge in homelessness, addiction, and mental health fields;
- Experience in case management, completing assessments, service planning, and goal setting;
- Experience working within a Housing First framework and an understanding of a variety of housing models including Housing First, Recovery-based; transitional, low barrier, supportive, etc.
KNOWLEDGE/SKILLS/BEHAVIORS
- Excellent knowledge of community resources related to homelessness, poverty, health, mental health, and substance use;
- Advanced working knowledge of housing resources available in the community;
- Understanding of the core principles that guide the work of a Housing First team;
- Demonstrated understanding of the unique issues and barriers affecting those experiencing poverty and homelessness;
- Comprehensive understanding of colonization and its impact on Indigenous peoples;
- Ability to work within a harm-reduction framework, including adopting a non-judgmental approach and distributing safer use supplies;
- Ability to work from a trauma-informed perspective;
- Demonstrated ability to conduct accurate and thorough assessments;
- Strong aptitude for matching interventions to stages of change appropriately;
- Excellent interpersonal skills and the ability to form relationships with a variety of populations, including volunteers, donors, stakeholders, and community members;
- Exceptional written and verbal communication skills;
- Highly developed understanding of and proven ability to communicate professionally and have appropriate boundaries;
- A compassionate, empathetic outlook, and the ability to convey these qualities in relationships with others;
- Demonstrated skill in de-escalation and mediation techniques;
- Strong organizational and time management skills, with the ability to effectively prioritize tasks under pressure and with competing demands;
- Proven critical thinking and problem-solving abilities;
- Proficient in Microsoft Office (Word, Excel, Outlook);
- Experience working with HIFIS4 is an asset;
RELATIONSHIP
- Building professional relationships with participants;
- Collaborate with other departments in the assessment and support of participants;
- Liaise, advocate, and collaborate with other agencies and housing providers throughout the sector;
WORKING CONDITION
- Hours of work: Monday – Friday (40 hours per week)
- Four shifts per week: 8 a.m. - 4:30 p.m.
- One shift per week: 11:30 a.m. - 8 p.m. (on the same designated weekday each week)
- Up to four overnight field trips per year;
- A work-issued cell phone will be provided. No on-call responsibilities are required;
- Must have a valid Class 5F driver’s license, an acceptable driver abstract, and a vehicle in good condition;
- Will be required to drive participants to appointments in personal vehicle;
- Will be required to assist participants with unit cleaning and preparation for pest control;
- May be required to lift up to 50 lbs. occasionally and assist participants with moving related to housing;
- Will require working alone with participants and visiting participants in their homes;
- Hazardous conditions could potentially include unpredictable or risky behaviours from community members;
