POSITION TYPE   PERMANENT, FULL-TIME

OVERVIEW: Reporting to the Memengwaa Supervisor, the Transitional Youth Worker provides case management supports to youth on an extension of care who are living in or transitioning out of Memengwaa transitional housing units. The key objective of this role is to support youth to build their personal capacity, in preparation for the next stages of their lives.

KEY RESPONSIBILITES:

  • Provide case management services to Indigenous youth in and from care.
  • Alongside team staff, screen referrals and complete intakes to the program.
  • Utilizing program tools, complete SMART goal setting and life skill assessments with tenants; follow up on goals and assessments ongoing.
  • Conduct weekly check-ins with youth to implement, review and update goals.
  • Refer to and liaise with appropriate resources; advocate for youth when required.
  • Collaborate with youth support to develop and provide comprehensive support based on identified needs.
  • Provide written reports to and work closely with SECFS case workers as required.
  • Maintain youth files by completing case logs, keeping copies of critical information, and filing information to the highest standards of confidentiality.
  • Attend meetings/appointments with youth when appropriate; empower youth to access resources independently.
  • Conduct suite checks and encourage tenant participation in household requirement, events and activities.
  • When youth move out of the program, provide ongoing case management support through check in and follow up.
  • Intervene in crisis situations as they arise in a calm and effective manner.
  • Collaborate with other Memengwaa staff to develop programming calendars.
  • Facilitate and/or coordinate programming for participants in the Memengwaa program, document attendance and complete workshop reports.
  • Recommend support staff schedules, in collaboration with the program manager.
  • Participate in team meetings & attend training as required.

QUALIFICATIONS:

  • Training in the human services field considered an asset, combination of work experience and lived experience will be considered.
  • Knowledge of the CFS system/Child Welfare required; knowledge of the experiences of youth aging out of care is a definite asset.
  • Understanding of how systems such as EIA, CFS, CLDS and other’s function; ability to support youth in navigating systems.
  • ASIST, NVCI, First Aid CPR, and other related training courses considered an asset.
  • Ability to work with addictions, mental illness, and trauma from a strengths-based perspective.
  • Strong knowledge of Community resources.
  • Excellent time management skills.
  • Possesses a strong knowledge of the Anishinaabe culture and issues affecting our children, families, and communities; obtained prior cultural awareness and/or sensitivity training. Ability to speak Anishinaabe an asset.
  • Excellent interpersonal and communication skills; ability to engage in active listening and build strong working relationships with youth.
  • Ability to prioritize tasks, with strong organizational skills.
  • Ability to handle conflict or crises in a calm and responsive manner.
  • Must have a valid class 5 driver’s license and access to a vehicle.
  • Must have Clear Criminal Record (vulnerable Sector Check), Child and Adult Abuse Registry Check, and Prior Contact Check and acceptable Driver’s Abstract are a condition of employment.
