POSITION TYPE PERMANENT, FULL-TIME
OVERVIEW:
Reporting to the Sacred Care Program Supervisor, the Sacred Care Case Manager is responsible for providing support to the Sacred Care homes. The Sacred Care Case Manager works within a team that serves to provide comprehensive case management services within a holistic circle of care.
KEY RESPONSIBILITIES:
- Establish a positive relationship with foster parents, youth in the homes, and the support team primarily through regular home visits and by ongoing communication by phone and via email.
- Conduct monthly case planning meetings in the foster homes with the assigned collaterals.
- Complete required Foster Care Licensing and service documentation.
- Maintain an accurate understanding of the services provided in each home and effectively communicate with the Program Manager and SECFS.
- Collect, review and distribute monthly reports and incident reports for each home. Assess concerns, strategize solutions and support homes in responding appropriately.
- Communicate to Program Manager the observed training needs for House Parents, Support and Respite Workers, and find those training opportunities.
- Ensure outcomes, assessments and service plans are current and up to date.
- Regular travel is required for this position within Winnipeg and surrounding areas.
- Ability to lift and carry items to a maximum of 50lbs.
QUALIFICATIONS:
- BSW degree or equivalent; similar education and experience working in the field will be considered.
- Knowledge in the areas of colonization and intergenerational trauma.
- Excellent conflict resolution skills.
- Knowledge of the Child and Family Services (CFS) sector and legislation.
- Understanding of foster care standards and related licensing paperwork.
- Demonstrate knowledge and/or understanding of First Nations perspectives, culture and philosophies.
- Experience developing and supporting a strong, respectful team-based approach to service delivery.
- Experience working with families and navigating complex situations.
- Clear Child Abuse registry, Adult Abuse registry, Criminal Record check with Vulnerable Sector search, and Prior Contact Check.
- Strong proficiency in Microsoft Office, word processing (MS Word), Spreadsheets (Excel), E-mail (Outlook) and Internet.
- Excellent communication, organization, problem solving and team skills.
- Ability to speak Anishinaabe is an asset.
- Must a valid Manitoba driver’s license and a reliable vehicle; satisfactory driver abstract provided annually.
- Must maintain a high level of confidentiality.
- Ability to work in a fast-paced environment.
- Must be resourceful with a high attention to detail.
- Ability to prioritize tasks and effectively manage multiple priorities.
