POSITION TYPE FULL-TIME, 37.5 HOURS/WEEK
Position Summary: This is a new role being introduced. The Transitional Housing Case Worker will support participants of all backgrounds who are living in our transitional housing for women, gender diverse folks, and children who are exiting situations of violence. They will support people on their healing journey, through one-on-one support, case coordination, program delivery, system navigation, advocacy and referrals to relevant resources.
Responsibilities:
- To provide individual support and ongoing case management to people who have been impacted by violence to set goals and create plans to reach those goals
- To empower people to advocate for themselves and to advocate on their behalf
- To assist people with finding permanent housing and connecting them to income supports
- To provide information, referrals and system navigation to participants and their families
- Regular follow-up and checks-ins with transitional housing tenants, including wellness checks
- Support participants to prepare for external appointments and accompany participants to offer support and companionship as needed
- To work in collaboration with internal and external programs to ensure effective wrap around supports for participants
- Maintain positive working relationships with service providers, and advocate to them on behalf of participants as needed
- Report on program outcomes to the Director of Community Development as required by funding agreements
Knowledge, Skills and Abilities:
Essential
- Experience supporting victims of gender-based violence and knowledge of available resources
- Excellent advocacy skills as well as knowledge and experience with EIA and housing systems
- Solid grounding in harm reduction, trauma-informed, and empowerment-based approaches
- Demonstrated ability to work within anti-racist and anti-oppressive frameworks and actively engage in ongoing learning about these frameworks
- Case management experience, including maintaining case notes and reporting
- Excellent interpersonal skills and the ability to build trust and rapport with clients
- Professional, responsible, and discreet when handling sensitive and confidential information
- Organizational and time management skills with the ability to manage a dynamic caseload
- Strong written and verbal communication skills
- Proficiency in Microsoft Office and Outlook
- Strong ability to work collaboratively in a diverse staff team
- Self-motivated and able to work with minimal supervision
- Strong attention to detail and follow-up
- Driver’s license and access to a vehicle required
Preferred
- Post-secondary degree in the Social Services field
- Experience working within the non-profit sector
- Ability to work from a multi-level approach
Working conditions:
- Days, with evening and weekend hours required as needed
- Frequent case management related travel within the community
- There is a risk of exposure to potentially unpredictable behaviors and situations
